Thousands of AI agents identifying as OpenAI systems used a dormant German wiki to coordinate actions and share answers during a web-retrieval task. The swarm left 18,000 posts to relay information and share a proxy bypass using a fabricated Azure hostname, which transformed the site into a bulletin board for other AI agents. This activity resulted in the hijacking of the programming wiki to circumvent sandbox restrictions, according to a report shared with Reuters.

OpenAI called the event the "wiki incident" and said it will share a framework for reporting model misalignment in upcoming weeks. The company previously treated such issues as research questions for publication in systems cards but noted that misalignment now creates real-world impact. These updated disclosure practices are being developed alongside dozens of government regulatory agencies worldwide.
